1. What Porta Health is
Porta Health is a personal health record application. It helps you record, organize, retrieve, and share your own health information — such as medications, conditions, allergies, immunizations, providers, and visit history.
2. What Porta Health is not
Porta Health does not provide medical advice, diagnosis, or treatment. Nothing in the app is a substitute for the judgment of a qualified healthcare professional. Always consult your doctor, pharmacist, or other provider about medical decisions, and never delay seeking care because of information in the app. If you think you may have a medical emergency, call 911.
3. Your account and your responsibilities
- You are responsible for keeping your login credentials secure and for the accuracy of information you enter.
- You may only connect records (yours or someone else’s) that you are legally authorized to access.
- You must be at least 18 years old, or use the app under the supervision of a parent or legal guardian.
4. Connecting external records
Porta Health can retrieve your records from healthcare organizations through patient-authorized connections (for example, logging in with your existing patient-portal account). Each connection happens only with your explicit authorization, which you can revoke. Retrieved records come from your healthcare organizations as-is; we do not alter their clinical content, and their accuracy remains the responsibility of the originating organization.
5. Your data
Your health information belongs to you. Our collection and use of information is described in the Privacy Policy. In short: we use your information to provide the service to you, we do not sell it, and you can export or delete your record.
6. Acceptable use
You agree not to misuse the service — including attempting to access records you are not authorized to access, interfering with the service’s operation, or using it to violate any law.
7. Disclaimers and limitation of liability
The service is provided “as is” during our development and testing period, without warranties of any kind, to the extent permitted by law. To the maximum extent permitted by law, Porta Health’s liability for any claim arising out of the service is limited to the amount you paid for it (currently: nothing).
8. Changes and termination
We may update these terms as the product develops; material changes will be posted here with an updated date. You may stop using the service and delete your account at any time.
